Application Form for 2017-2018 Fellowship Period

INSTRUCTIONS: Please complete the form below. You must complete this in one sitting.  At the beginning of the form you will be asked to upload a project summary (approximately 250 words), a project description (approximately 5 pages), a curriculum vitae, and a sample of work (you may also provide a link to your work, if you prefer). Each file must be less than 10 MB. NOTE: If you receive an error message during the upload process, you may leave these fields blank and email your three files as attachments to du_bois@fas.harvard.edu.

Three (3) letters of recommendation are required. They may be emailed to du_bois@fas.harvard.edu, or sent via postal mail to the following address:
